Account Deletion Policy

How account deletion requests are reviewed, verified, and handled.

Wells Legacy Health & Wellness provides this Account Deletion Policy to explain how patients, website visitors, and users may request deletion or deactivation of certain online account information, while also explaining why medical records, billing records, legal records, and required healthcare documentation may need to be retained.

Policy Overview

Account deletion is different from healthcare record deletion.

This policy explains what may be deleted, deactivated, limited, retained, or referred to a third-party platform when a patient or user requests removal of online account information connected to website tools, appointment links, forms, communication preferences, or third-party services. In-person primary care in Altamonte Springs remains the primary focus of the practice, with telehealth available in Florida and Minnesota when clinically appropriate for eligible patients.

Online Account

Account access removal

Eligible account deletion requests may involve deactivating login access, removing profile information, closing a digital account, or limiting future account-based communication.

Healthcare Records

Required retention

Medical records, billing records, appointment documentation, consent records, privacy records, and compliance records may need to be retained as required or permitted by law.

Verification

Identity confirmation

Requests may require reasonable verification before account information is deleted, deactivated, updated, restricted, or referred to a third-party service provider.

This policy applies to requests to delete, deactivate, close, restrict, or remove certain online account information connected to Wells Legacy Health & Wellness website tools, appointment links, communication preferences, forms, digital resources, and third-party platforms used to support practice communication.

This page should be read with the Web & Mobile Privacy Policy, Notice of Privacy Practices, Information Security & Data Protection, Terms of Service, and Legal, Privacy & Compliance pages.

Depending on the request, applicable law, verification status, account type, and technical limitations, certain non-required account information may be eligible for deletion, deactivation, restriction, or update.

Examples may include:

  • Website account login access, if applicable.
  • Optional profile information not needed for care, billing, compliance, security, or legal purposes.
  • Non-required communication preferences.
  • Non-essential website account settings.
  • Optional newsletter or marketing preferences, if used.
  • Duplicate or outdated non-medical website account information.

Account deletion does not automatically delete protected health information, medical records, billing records, appointment records, payment records, consent records, communication records, compliance records, security logs, legal records, or other documentation that Wells Legacy Health & Wellness must or may retain.

Records may be retained for reasons including:

  • Healthcare treatment, payment, and healthcare operations.
  • Medical record retention and continuity of care.
  • Billing, accounting, tax, insurance, or audit requirements.
  • HIPAA, privacy, security, and compliance obligations.
  • Legal defense, dispute resolution, fraud prevention, or safety review.
  • Identity verification, security monitoring, and unauthorized access prevention.
  • Required documentation for patient rights requests or prior account activity.

Patients may have rights to access, amend, or request restrictions related to certain health information, as explained in the Notice of Privacy Practices.

Wells Legacy Health & Wellness may use third-party platforms for appointment booking, telehealth, forms, payment processing, email, analytics, website hosting, review links, maps, social media, or other operational services.

Some account deletion requests must be handled directly through the third-party platform because Wells Legacy Health & Wellness may not control that account, login, platform profile, stored credentials, device data, or third-party privacy settings.

If a request involves a third-party platform, the practice may provide general direction, update information within its control, or refer the user to the platform’s own privacy or account deletion process.

Wells Legacy Health & Wellness may require reasonable identity verification before taking action on an account deletion, deactivation, restriction, or information request. Verification helps protect patients and users from unauthorized account changes, privacy risks, fraud, or improper access.

If a request is submitted by a caregiver, parent, legal guardian, personal representative, attorney, or other authorized person, additional documentation may be required before the practice can act on the request.

The practice may decline, delay, or limit a request when identity cannot be verified or when the requester does not have authority to act on behalf of the account holder or patient.

Wells Legacy Health & Wellness aims to review account deletion requests within a reasonable period after receiving enough information to understand the request and verify the requester’s identity.

Some requests may require more time when the account involves third-party systems, healthcare records, security review, legal retention obligations, billing records, patient identity verification, or multiple systems.

The practice may respond by confirming completion, explaining what could not be deleted, requesting additional verification, providing alternative options, or referring the requester to a third-party platform.

Wells Legacy Health & Wellness may deny or limit deletion requests when deletion would conflict with healthcare obligations, legal requirements, medical record retention, billing or accounting requirements, security needs, fraud prevention, patient safety, legal claims, compliance obligations, or system backup limitations.

This policy does not create a right to delete medical records or other protected records when retention is required or permitted. It also does not replace patient rights described in the Notice of Privacy Practices.

This website, email, contact forms, online booking, and account deletion request channels are not for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.

Wells Legacy Health & Wellness may update this Account Deletion Policy from time to time. The updated version will be posted on this page with a revised “Last updated” date.

Continued use of the website, online resources, appointment links, or related digital tools after updates are posted means the updated policy applies going forward.
