Policy Overview

Ethical reporting supports safer, more accountable healthcare communication.

This page explains how patients, visitors, vendors, community members, and workforce-related individuals may raise good-faith compliance or ethics concerns and how those concerns may be reviewed, documented, routed, and addressed.

Report Concerns

Good-faith reporting

Concerns about privacy, billing, safety, fraud, waste, abuse, discrimination, accessibility, or improper conduct may be reported for review.

Non-Retaliation

Protected communication

Wells Legacy Health & Wellness does not support retaliation against individuals who raise compliance or ethics concerns in good faith.

Routing

Appropriate review

Concerns may be reviewed internally, documented, escalated, or directed to external agencies when appropriate or required by applicable law.

This policy applies to good-faith concerns related to Wells Legacy Health & Wellness services, website communication, patient communication, appointment access, in-person care access, telehealth when clinically appropriate, privacy practices, compliance practices, billing-related concerns, safety concerns, and professional conduct.

This page should be read with the Notice of Privacy Practices, Information Security & Data Protection, Responsible Disclosure, Nondiscrimination and Accessibility, and Legal, Privacy & Compliance pages.

Examples of concerns that may be reported include:

  • Privacy, confidentiality, or patient information concerns.
  • Suspected fraud, waste, abuse, improper billing, or misuse of resources.
  • Safety concerns involving patient care communication or practice operations.
  • Discrimination, accessibility, language assistance, or retaliation concerns.
  • Conflicts of interest, unethical conduct, or professional conduct concerns.
  • Cybersecurity, phishing, suspicious messages, or website security concerns.
  • Concerns involving compliance with applicable healthcare rules or practice policies.

To report a non-emergency compliance or ethics concern to Wells Legacy Health & Wellness, use the Contact page, call (321) 306-6755, or email info@wellslegacyhealth.com.

When reporting, provide as much useful detail as possible, such as:

  • The type of concern.
  • The date, time, location, or communication method involved.
  • Names or roles of people involved, if known.
  • A short description of what happened.
  • Any non-sensitive supporting details that may help the review.
  • A safe way to contact you for follow-up, if you want a response.

Do not send highly sensitive medical information, passwords, payment card information, or unnecessary personal details through public social media or unsecured channels.

Reports may be reviewed by appropriate Wells Legacy Health & Wellness personnel or designees based on the nature of the concern. Information may be shared only as needed to review, document, investigate, respond, or comply with applicable legal or regulatory obligations.

Wells Legacy Health & Wellness cannot guarantee complete anonymity when follow-up, investigation, documentation, legal obligations, patient safety, or regulatory reporting requires disclosure. However, the practice aims to handle concerns with discretion and appropriate privacy safeguards.

Wells Legacy Health & Wellness does not support retaliation against individuals who raise compliance, ethics, privacy, accessibility, nondiscrimination, safety, or billing concerns in good faith.

A good-faith report means the person reasonably believes the information reported may be true or may require review, even if the concern is later found to be unsubstantiated.

Knowingly false reports, threats, harassment, misuse of reporting channels, or reports made for improper purposes may be handled separately based on the circumstances and applicable law.

This page, website, email, contact forms, online booking, social media, and routine practice communication channels are not for medical emergencies, crisis situations, or immediate safety threats.

If you are experiencing a medical emergency, life-threatening symptoms, immediate danger, abuse, violence, suicidal thoughts, or another urgent safety issue, call 911 or go to the nearest emergency room immediately.

Some concerns may also be reported to external agencies depending on the issue, service type, patient location, and applicable law.

Examples include the HHS Office for Civil Rights for certain civil rights or health information privacy concerns, the HHS Office of Inspector General for certain fraud, waste, and abuse concerns involving HHS programs, the Florida Agency for Health Care Administration, or the Minnesota Department of Health.

Wells Legacy Health & Wellness may update this policy from time to time. The updated version will be posted on this page with a revised “Last updated” date.

Continued use of the website, online resources, appointment links, or related digital tools after updates are posted means the updated policy applies going forward.
