Important conditions for appointments, services, communications, and website use.

These Terms and Conditions provide additional details about Wells Legacy Health & Wellness, where the dominant care focus is in-person primary care in Altamonte Springs, with telehealth available in Florida and Minnesota when appropriate. This page also explains appointment requests, patient responsibilities, communications, third-party tools, website use, and related policy expectations.

Terms Overview

These conditions support safe, appropriate, and respectful use of services.

These Terms and Conditions should be read together with the Terms of Service, Web & Mobile Privacy Policy, Notice of Privacy Practices, Legal Disclaimer, and other policies listed in the Legal, Privacy & Compliance hub.

Appointments

Scheduling is subject to confirmation.

Appointment requests, online booking, and portal activity may require confirmation, eligibility review, accurate patient information, and practice acceptance.

Telehealth

Telehealth is secondary and has limits.

Telehealth may be available in Florida and Minnesota when appropriate for eligible patients, but in-person primary care in Altamonte Springs remains the dominant focus. Urgent care, emergency care, labs, or specialist care may be recommended.

Communication

Use appropriate contact methods.

General website forms, regular email, and social media should not be used for emergencies, urgent symptoms, or sensitive medical details.

By using this website, requesting an appointment, accessing online resources, submitting information, using contact options, or interacting with Wells Legacy Health & Wellness digital tools, you agree to these Terms and Conditions.

These Terms and Conditions supplement the Terms of Service. If you do not agree, do not use the website, online forms, appointment links, or related digital resources.

Wells Legacy Health & Wellness may provide services such as personalized primary care support, telehealth, physical exams, acute care support, urgent care guidance, chronic disease management support, health and wellness education, patient advocacy, health and wellness coaching, CPR training, First Aid training, and community-focused health education.

Service availability may vary based on patient eligibility, provider availability, scheduling, state location, clinical appropriateness, technology access, insurance or payment requirements, and applicable law. Not all services are appropriate for every patient or concern.

Online booking and appointment requests are not guaranteed until confirmed through the scheduling system or by Wells Legacy Health & Wellness. The practice may need additional information before accepting, confirming, rescheduling, or canceling an appointment.

Patients are responsible for:

  • Providing accurate contact, demographic, insurance, payment, and health-related information.
  • Keeping appointment times or contacting the practice as early as possible when rescheduling is needed.
  • Completing required forms, consents, portal steps, and intake information before the appointment when requested.
  • Using the correct appointment type and selecting a service that matches the patient’s concern as closely as possible.
  • Seeking emergency or urgent in-person care when symptoms require immediate attention.

Wells Legacy Health & Wellness may update scheduling, cancellation, no-show, payment, or appointment policies at any time and may communicate specific requirements before or during scheduling.

Telehealth may be available for eligible patients in Florida and Minnesota when the requested service is clinically appropriate and permitted by applicable law. Telehealth is not suitable for every condition, symptom, diagnosis, treatment plan, or patient situation.

Patients using telehealth may be required to:

  • Confirm their identity and current physical location at the time of service.
  • Use a private, safe, and appropriate environment for the visit.
  • Use reliable internet, a working camera, a working microphone, and an appropriate device.
  • Complete required consents, forms, and portal steps before the visit.
  • Seek in-person care, urgent care, emergency care, labs, imaging, or specialist care when recommended.

If the provider determines that telehealth is not clinically appropriate, Wells Legacy Health & Wellness may recommend another care setting or service option.

This website, online booking, general email, contact forms, social media, and routine digital communications should not be used for medical emergencies or urgent symptoms.

If you are experiencing chest pain, trouble breathing, stroke symptoms, severe allergic reaction, uncontrolled bleeding, suicidal thoughts, severe pain, sudden confusion, loss of consciousness, or any life-threatening condition, call 911 or go to the nearest emergency room immediately.

Wells Legacy Health & Wellness may communicate through phone, voicemail, email, text, patient portal, telehealth systems, online booking platforms, mail, or other communication methods as appropriate.

Patients are responsible for keeping contact information current and understanding that standard email, text messaging, and public website forms may not be secure enough for sensitive medical information unless the practice specifically identifies a secure method.

Response times are not guaranteed through general contact methods. Do not use these channels for emergencies or urgent medical needs.

Patients are responsible for understanding insurance benefits, plan limitations, coverage, copays, deductibles, coinsurance, prior authorization requirements, out-of-network status, self-pay amounts, and any financial responsibility communicated by the practice or payer.

Insurance verification or benefit information is not a guarantee of payment or coverage. Final payment responsibility may depend on the patient’s insurance plan, payer rules, claim processing, medical necessity, service type, documentation, eligibility, and applicable policies.

Wells Legacy Health & Wellness may use third-party systems for payment processing, billing, insurance verification, claims, statements, collections support, or administrative operations.

Patients may be asked to complete intake forms, consent forms, privacy acknowledgments, telehealth consents, medical history forms, payment forms, insurance forms, or other documents before services are provided.

Patients are responsible for providing accurate, complete, and updated information. Inaccurate or incomplete information may affect scheduling, clinical decision-making, billing, insurance processing, communication, or service availability.

Requests related to medical records, corrections, restrictions, disclosures, privacy rights, or other patient record matters should be directed to Wells Legacy Health & Wellness using the appropriate contact method.

Website visitor information is addressed in the Web & Mobile Privacy Policy. Protected health information used or disclosed for treatment, payment, healthcare operations, and other healthcare-related purposes is addressed in the Notice of Privacy Practices.

General website forms, regular email, and social media should not be used to send highly sensitive medical details unless Wells Legacy Health & Wellness specifically directs you to an approved secure method.
